Why AI does this. In plain terms: it is built to sound convincing whether or not it knows, so it fills the gaps in your notes with confident guesses.
The two-part fix. An objective definition of what a good action item is, and an independent reviewer that checks the draft against it.
The six-point bar. The exact criteria an action item has to meet: grounded, owned, time-bound, actionable, complete, and sorted.
The ready-to-use grader. A file you paste into Claude or ChatGPT with your notes. It drafts, then grades its own work as a skeptic, and fixes it until it is clean.
